Your First Appointment

You will need to bring your drivers license and insurance card to your first appointment so that it may be copied and placed in your chart. The doctors at Texas Fertility Center like to review all previous medical records pertaining to your obstetrical and gynecological history. Please click on the link below to print out a copy of our medical records release in order to have your records sent to our office by your appointment date.
