To schedule an
appointment for a semen analysis, please follow these steps:
Obtain a semen
analysis requisition form from your doctor.
If you are a
patient of TFC, you will be given an order form with instructions to
call AIVF to schedule an appointment.
If you are using
a physician outside of TFC, your doctor can print a copy of the
order form by going to www.austinivf.com or by having their office call 512-610-7474 to
receive a fax copy.
Your doctor does
not need to send in the requisition form prior to your arrival. You
may bring this form with you to your scheduled appointment.
Schedule your
appointment
Once you have obtained a requisition
form, please call Austin IVF at 512-610-7474 to schedule your
appointment. The phones are answered Monday through Friday, from
8:30am to 4:00pm. You MUST have an appointment to drop off a
sample.
Austin IVF does
not see patients without an appointment.

When you
arrive
You will need to present your
driver’s license to the receptionist. Photo identification is
required at the time of specimen drop off, and specimens will only
be accepted from the patient or documented partner.
Austin IVF is not contracted with
any insurance companies, and payment in full is due at the time of
service. A receipt can be provided in order to submit for
reimbursement from your insurance company.
*Note: It is recommended that you
abstain from ejaculation for 2-3 days prior to your visit, unless
your physician has made other recommendations
Sample
Collection
Home collection In
most cases, you may collect a semen sample at home to bring to your
appointment. If you choose to collect at home, you will need to
obtain a special specimen container in advance from Austin IVF or
your referring physician’s office. Also, please arrive with your
specimen having been collected no more than 60 minutes prior to your
appointment time. If you will not be able to arrive 60 minutes after
you have collected your specimen, you may consider collecting on
site at Austin IVF, or rescheduling your appointment. In the event
that you have collected your specimen and it will be over 60 minutes
old, please call Austin IVF for information on how to best proceed.
We have two drop off locations. The staff at AIVF will work with
you to coordinate the location, date and time of your appointment.

Collection instructions
*Note: It is recommended that you abstain from ejaculation for
2-3 days prior to your visit, unless your physician has made other
recommendations.
Wash your hands before and after collection with a mild soap. Please
dry your hands well before collection.
The sample is collected by masturbation into a sterile container
provided by your physician or Austin IVF.
A lubricant can also be provided upon request that is safe for
sperm. DO NOT USE CONTAINERS FROM HOME. DO NOT USE LUBRICANTS,
COMMERCIAL CONDOMS, SOAPS, LOTIONS, OR BODILY FLUIDS SUCH AS SALIVA.
The entire ejaculate must be collected. If the semen specimen has
spilled or has been incompletely collected, notify the staff at
specimen drop-off. After collection, the specimen must be kept at
room temperature and protected from sunlight exposure until it is
delivered to the laboratory.
The specimen cup must be labeled completely before it is delivered
to the lab. Labels are available at the lab reception desk.
Specimen Containers must be labeled with the following:
Patient’s first and last name, date of birth, and Social
Security number
Also partner’s first and last name and date of birth
Days abstained
Time of collection
Whether a portion of the sample was lost during collection
