Registration Process


An initial appointment can be scheduled at Texas Fertility Center by calling (512) 451-0149 X 7449 and speaking with one of our New Patient Coordinators. If your appointment is for infertility, we strongly encourage scheduling a time for your initial visit when both you and your partner are available. Once the appointment has been scheduled, our New Patient Coordinators will work closely with you to complete the registration process.   As part of this process, a  New Patient Information Booklet, will be mailed to you.  Alternatively, you can download it from our website.   

A New Patient Coordinator will confirm your insurance coverage and ask you specific questions to determine the extent of that coverage.  In order for the coordinator to verify this information and to provide an estimated cost for your initial visit, we will need some information on both you and your partner.  We ask that you complete and return our Patient Information and Insurance Information forms to us at least 7 days prior to your scheduled appointment.  For your convenience, these forms can be submitted directly from our website Patient Information (Online) and Insurance Information (Online). If you prefer, they can also be downloaded and then either faxed to (512) 451-0977 or mailed to:

You will be contacted prior to your initial visit to review your insurance coverage, to go over the estimated cost for your first visit, and to answer any questions you may have in preparation for your appointment.  We encourage patients to also independently verify their coverage, and we have provided an insurance benefit questionnaire that can be downloaded from our website to assist in talking with your insurance company.  If the insurance company requires a referral to Texas Fertility Center, you must obtain it before being seen.  This can be done by contacting your primary care physician.   Payment in full will be due at the time of service if our office is not a contracted provider with your insurance company or if you do not have insurance coverage for the visit.  Questions regarding insurance benefits can be discussed with a New Patient Coordinator at (512) 451-0149 x 7420 or by emailing your questions to: newpatient@txfertility.com.  Please be aware that neither the verification of benefits that you obtain from your insurance company, nor the verification that we obtain, guarantees payment by the insurance provider. 

If your appointment is for infertility or if you are attempting pregnancy, both you and your partner are strongly encouraged to be at the initial visit. According to the Texas Medical Practice Act, we cannot prescribe medications or order tests (semen analysis, antisperm antibodies, hormone levels, infectious disease screening, etc.) on patients without first at least taking their medical history.  Having both partners present at the initial visit will therefore avoid any unnecessary delays in the diagnostic and treatment processes.  Please note that there is an additional fee for the evaluation and management services provided to the male partner.  Our New Patient Coordinator will verify insurance coverage to determine if this service will be covered by either the female or male partner’s insurance. If your insurance company requires a referral prior to being seen, please contact your primary care physician, as this documentation must be obtained before your initial visit.

A copy of any relevant medical records (i.e. previous infertility testing, treatment or surgeries that either you or your partner may have had) should be sent to Texas Fertility Center prior to your initial visit. An Authorization for Release of Confidential Information (To TFC) can be downloaded from our website to expedite this process. In order to insure that your initial visit is as productive and informative as possible, please submit this request for records to your previous physician(s) at least 15 business days before your initial appointment with our office.

Upon arrival at your initial appointment, we will ask you to review and sign a copy of our Acknowledgement of Review of Notice of Privacy Practices, Financial Policy, and Authorization for Disclosure of Confidential Information which can all be downloaded from our website along with our New Patient Information Booklet.   Our office will need to make a copy of a government-issued photo ID and insurance card for both you and your partner at this visit.

General Information

  • Our regular office telephone hours are 8:30 a.m. to 12:00 p.m. and 1:30 p.m. to 4:00 p.m., Monday through Friday.

  • Our office hours are 7:30 a.m. to 12:00 p.m. and 1:30 p.m. to 5:00 p.m., Monday through Friday.

  • In order to be fair to all of our patients, we see patients only by appointment, with obvious exceptions made for true medical emergencies.

  • Our physicians are available to see established patients who must be seen on weekends and on holidays. We will announce any available office hours for the upcoming weekend and/or holiday every Friday on our after-hours telephone message.  This message is updated weekly.

  • As our physicians only see a limited number of new patients each week, our policy requires you to secure your new patient appointment time with a deposit via credit card or personal check/money order. If you cannot keep your appointment, please cancel or reschedule at least 48 hours in advance of your scheduled appointment to avoid forfeiting your deposit.
