Medical Records



In order for you to obtain a copy of your medical records, an authorization must be completed and signed.  The form, Authorization for Relase of Confidential Information (From TFC), can be downloaded from our website.  The completed form can be mailed to our office or faxed to (512) 451-0977.  Please submit the form with attention to Medical Records.  All requests will be processed within fifteen (15) business days after receipt of a proper written request.  In compliance with Texas statute and according to the rules set by the Texas State Board of Medical Examiners, we charge a fee of $25 for the first 20 pages and $.50 per page thereafter, plus postage, for a copy of your records.   

Patients who want to have any records sent to Texas Fertility Center can request those records by completing the Authorization for Release of Confidential Information (To TFC), which can be downloaded from our website and mailed to your previous physician. 

How do I obtain a copy of my medical records?

How long does it take to obtain my records?

Is there a cost to obtain my records?

Will the physician need to see my previous medical records at my new patient appointment?

Yes.  A copy of your medical records that pertain to any previous infertility testing, treatment or surgeries that you or your partner may have had should be sent to our office at least one week prior to your visit.  The form to obtain your records can be downloaded from our website.
